An exciting
vacancy has arisen for a motivated and enthusiastic Administrator to support Virtual
wards and urgent care response (HICAF). You should be highly committed, organized
and be able to work to a high standard. The ability to prioritize, work to deadlines
and achieve targets is also essential and a willingness to support change and
improvement within the service is expected. The Intermediate Care and Frailty
Service is committed to delivering an excellent service to all patients both in
the Community and Care Settings. This is
a Health and Social Care multi-disciplinary/multi-organisational service,
Experience of working within a healthcare/social care
environment is desirable but not essential however it is essential that that
the applicant should demonstrate excellent verbal and written communication
skills with clients, carers and other professionals. It is essential that the applicant has the
ability to travel independently across the Trust and be able to work flexibly
over 7 days on rota.
Main duties of the job
The role consists of
various duties including liaison with Health Care Professionals, working across
multi-disciplinary, multi-organisational Health and Social Care Services,
managing referrals on the Health and Social Care databases, general office typing,
telephone enquiries & liaison with service users.

Job Purpose:
To provide
comprehensive administrative support to the Hicaf team based at Runcorn town
hall (HICAF)working flexibly over 7 days as required by rota.
Duties and Responsibilities:
To provide personal administration and
secretarial support for the Intermediate Care and Frailty Service to ensure the smooth and
efficient running of services.
To maintain a high professional standard in
terms of the performance of duties in terms of general conduct, including
maintaining confidentiality.
To work flexibly across organisational
boundaries as required and to present at all times a positive image of the Services.
To prioritise workload to meet agreed timescales.
To take and transcribe minutes of approved Service/Team
meetings ensuring they accurately reflect the discussions at the meetings.
To collate data for Commissioners and Clinical
audit and generate reports using appropriate office applications mainly EMIS.
To maintain electronic diaries and arrange appointments
and to deal with any correspondence relating to this and log all messages received.
Typing of clinical letters/medical reports and
other correspondence required by the Services.
To be responsible for sourcing, ordering and
auditing of resources.
To be responsible for the requesting of medical
documentation from other hospitals / units as and when required.
Work across multi-disciplinary, multi-organisational
Health and Social Care Services.
Load referrals and input data on Social Care
Database.
Communication
To liaise and maintain excellent communication
skills, both written and verbal, at all levels within the team.
To organise meetings as directed. These may require contact with multi agencies
requiring the co-ordination of mutually convenient times.
To assess and action information received, using
own initiative and to refer on to appropriate individual as necessary.
To be first point of contact for clients and
offer a professional solution to client queries.
To be able to communicate effectively and
professionally both on the telephone and face to face with clients, carers and
other professionals.
