Are
you an experienced nurse looking for the next step in your career? As a cluster
lead, alongside your community matron, you will be responsible for the staff
nurses and clinical assistants in your cluster as well as assessing, planning
and delivering patient centred programmes of care.
The NHS long term plan is to provide more care in community settings such as
patients in their own homes whilst enabling early discharge from hospital and
reducing hospital admissions.

Neighbourhood services operates from 7am to 10pm every day of the year with a

Main duties of the job
You
will be a registered nurse and have excellent holistic assessment, planning and
evaluation skills. You will use your advanced practice skills to deliver
planned and unplanned care for the complex patients on your caseload.
Along with a community matron, you will lead staff nurses and clinical
assistants delivering high quality care. You will hold appraisals for your
cluster and support staff with clinical supervision. Attending meetings such as
clinical case managers meetings to discuss clinical updates are an important
part of your role.
Acting as shift lead in a multi-disciplinary team means you will be first point
of contact for a variety of professionals and provide clinical support to staff
during the daily handover. Liaising with colleagues in the triage hubs and
external partner organisations you will ensure unplanned care is delivered
efficiently and effectively. Through the allocation process you will facilitate
planned care for the patients on your caseload.

The post holder will be responsible
for their own caseload within their speciality area as an autonomous
practitioner. In doing so, they will assess, plan, deliver and evaluate patient
centred programmes of care ensuring that they practice within their sphere of
competence and knowledge. The post holder will work to professional and
regulatory body codes, standards and guidance at all times ensuring that their
practice is grounded in evidence based theoretical and practical knowledge. The
post holder will provide the day to day line management of a team of
clinicians.

undertaking clinical practice at a frequency agreed with Service
Manager/Team Manager/Senior Practitioner Pathway Lead in the relevant field.

service delivery, responding to social, scientific, clinical and ethical issues
in line with the values and agreed model within the service

Works to standards of proficiency identified by
their professional and regulatory bodies

To
comply with the organisations Infection Prevention and Control requirements,
including bare below the elbows dress code for staff with a direct care
delivery role

structure specific to service area
2.3. Demonstrates the ability to delegate,
