The Neighbourhood Night Service operates from 20:45 07:15, seven days per week. We strongly recommend getting in touch to discuss the role further.

Main duties of the job

You will be a registered staff nurse and have excellent assessment, planning and evaluation skills to deliver clinical care to patients and support our Nursing Assistants overnight.

You will clinically coordinate and manage care ensuring that all calls are effectively assessed, triaged, allocated and recorded. You will have an overview of the service as a whole to ensure that patients and staff remain safe and care is appropriate and effective overnight. E.g. re-allocating work to cover unexpected staff absence.

You must have excellent communication, organisational and IT skills as you will be coordinating care for a large number of complex patients.

You will provide clinical leadership, including line management of Band 5 Staff Nurses, and oversee line management of our Band 3 Nursing Assistants.

You must be able to think on your feet, problem solve and juggle conflicting priorities as well as motivate others in challenging times and utilise the resources available to you effectively.

The Community Neighbourhood Nursing Night Service operates over a 24 hour period, providing holistic high quality domiciliary nursing care to patients registered with a Leeds General Practitioner. The service delivers care overnight as part of the Health and Social Care Team within each Neighbourhood.

The service aims to reduce hospitalisation and promote discharges through robust multi-disciplinary working with internal services, external services / organisations and the third sector. The service supports palliative care patients to die in their preferred place of care.

The key working relationships include the Neighbourhood Teams Clinical Leads, Joint Care Management, external agency providers, Marie Curie, Commissioners and Administrative Staff.

Key Responsibilities

1. Clinical

1.1. Demonstrates a detailed theoretical and practical knowledge of common aetiologies, pathologies, conditions and presentations involving a critical understanding of theories and principles

1.2. Demonstrates innovative and specialist skills to select methods and tools to holistically assess, plan, implement and evaluate in a complex and specialised field of work.

1.3. Demonstrates highly developed physical skills where dexterity and accuracy are important e.g. syringe pumps and infusions, manual treatments

1.4. Makes consistent judgements requiring complex fact analysis, interpretation and comparison of options and ensures clinical reasoning is reflected accurately in case records

1.5. Engages and actively involves the individual and, if appropriate, their family/carer, in the assessment, planning, implementation and evaluation of programmes of treatment and/or care by using person-centred techniques to promote a culture of involvement and empowerment
