Job summary

We have an exciting opportunity for two motivated, enthusiastic & hard working physiotherapists to join our team. The Community Stroke Rehabilitation Team offers specialist multi-disciplinary rehabilitation to adults who have had a stroke.

The service provides goal focused, time limited rehabilitation in patients home environment and other community settings. The team work closely alongside colleagues from Leeds Teaching Hospital Trust to support patients along the stroke pathway, from hospital to home.

This is an exciting time for service development as we work towards implementing the new National Stroke Service Model and the National Stroke Clinical Guidelines.

We are looking for an innovative, caring physiotherapist to join our friendly & flexible team. You will have access to regular clinical supervision, in-service training and peer support from highly skilled clinicians in the team but also the wider neurological service and stroke pathway.

Some weekend working is required.

Main duties of the job

You will have the opportunity to:

Carry out holistic multidisciplinary assessments

Carry out specialist physiotherapy assessments and treatment interventions

Work collaboratively with MDT colleagues within the team

Work in an integrated way with colleagues from Neighbourhood teams, local acute Trusts and social care

Access regular clinical supervision, appraisals, specialist training and peer support from staff throughout the stroke pathway

Participate in service development, innovation projects and audits

Develop your leadership skills by supervising and delegating to other staff including non-registered staff

Due to the nature of the role it is essential that applicants are able to travel independently across the locality and have access to a suitable vehicle for business purposes. If necessary adjustments can be considered in accordance with the Equality Act 2010.

Job Purpose

The post holder will be responsible for their designated workload within their speciality area with access to the support, advice and management from a senior clinician whilst not directly supervised. In doing so, they will assess, plan, deliver and evaluate patient centred programmes of care ensuring that they practice within their sphere of competence and knowledge. The post holder will work to professional protocols and regulatory body codes, standards and guidance at all times ensuring that their practice is grounded in the best available evidence based theoretical and practical knowledge.

1.5. Engages and actively involves the individual and, if appropriate, their family/carer, in the assessment, planning, implementation and evaluation of programmes of treatment and/or care by using person-centred techniques to promote a culture of involvement and empowerment

1.6. Works to standards of proficiency identified by their professional and regulatory bodies performing clinical treatments/procedures to a competent standard
