In this role you will work in one of our 13 Neighbourhood Teams based at Yeadon
Health Centre, who operate from 07:00 - 22:00 providing care for patients
requiring intervention in their own homes. The Neighbourhood Teams are part of
a wider multi-disciplinary team to deliver 24/7 nursing care for patients in
the community.
This is an exciting role that incorporates many different aspects of patient
care delivered to a wide range of patients.
We will support you with a fully tailored induction programme and will
encourage and facilitate on-going development, including the opportunity to
develop and learn skills from team members from a variety of professions.
Main duties of the job
You
must have excellent communication, organisational and IT skills.
You will need to assess and respond to the changing needs of the patient,
working within your own limits and seeking support from the wider clinical team
where appropriate.
The contract is for a minimum of 24.5 hours per week up to a maximum of 37.5
hours per week and have the flexibility to rotate through all the shift
patterns of 8:00 - 16:00, 10:00 - 18:00, 14:00 - 22:00 over 7 days per week
including bank holidays
Please note this role does require a full Driving licence and use of your own
vehicle which is essential to this job role.

The post holder will be providing
direct and indirect patient care independently but with support available from
a supervisor within the clinical area. In doing so, they will support the
assessment of service users and implementation of programmes of care delegated
to them by the appropriate staff member ensuring that they practice within their
sphere of competence and knowledge. The post holder will work within a range of
procedures and pathways in which they have received training in order to be
competent.
