Job summary
An exciting opportunity has arisen for a full-time Band 5 Cardiac Nurse within the Leeds Community Cardiac Service.
The Leeds Community Cardiac Service is made up of specialist nurses, physiotherapists, occupational therapist, and clinical support workers providing care and treatment for cardiac patients. You will support the Cardiac Nurse Specialists providing holistic assessments for patients predominantly with heart failure.
We are looking for passionate, dynamic and flexible individuals who are able to adapt to changing demands and workload. You will enjoy working as part of a multi-disciplinary team, be a good team player, but also be able to work independently.
The Cardiac service operates Monday to Friday from 8:30am to 4:30pm. Due to the nature of the role, it is essential that you are able to travel independently across the city and have access to a suitable vehicle for business purposes.
Main duties of the job
As a Cardiac nurse you will:
- Work within the MDT, particularly alongside Cardiac Nurse Specialists to provide support, care and advice for patients following myocardial infarction and cardiac surgery.
- Contact patients following discharge from hospital, completing a holistic assessment and discussing cardiac risk factors.
- Put the patient at the heart of everything you do, supporting the patient following discharge, completing the cardiac risk factor assessment and using the patients goals to identify a plan of care.
- Liaise with the Cardiac Activity Programme team, including cardiac physiotherapist to optimise the cardiac rehabilitation goals.
This role will include telephone/video consultations, home visits and clinic work. Training and education will be given to support you to develop cardiac rehabilitation skills.
