Job summary

Central North Leeds Primary Care Network (PCN) are looking to recruit a clinical pharmacist, to work at the heart of the PCN, delivering excellent pharmacy-led services to our population. We are looking for a highly motivated and aspirational clinical pharmacist to join our established team, to work closely with the multidisciplinary teams across the PCN and the wider locality. Central North Leeds PCN is made up of 5 practices covering a population of 81k.

This role is critical in supporting the delivery of the Primary Care Network Contract DES specifications and will help lead improvements to maximise safe, evidenced-based best practice to improve the quality of patient care.

Applications are welcome from pharmacists with patient-facing experience from any sector. An interest in working in general practice is essential. As a clinical pharmacist you will receive fully accredited training from CPPE and will be provided with appropriate supervision from a GP clinical supervisor and from senior pharmacists.

Main duties of the job

You will work within your clinical competencies, with a high level of support and supervision from the PCN Senior Pharmacists, to:

  • Undertake clinical medication reviews with patients and produce recommendations on prescribing and monitoring. These reviews could be cohort based, in care homes, polypharmacy or any other area required by the PCN
  • Deal with management of medicines on transfer of care
  • Work with practices enabling them to develop and apply systems for safer prescribing
  • Develop and deliver medicines-related training for primary care colleagues
  • Connect and communicate well with people who take medications, keeping them at the heart of the service you offer and enabling them to understand and manage their condition and their medicines effectively for themselves
  • Address health inequalities and proactively reach people from all communities
  • Participate in the PCN MDT

  • May hold or be working towards an Independent Prescribing qualification
