Leeds
Community Healthcare NHS Trust is the leading mental health provider of
Childrens and young peoples mental health services (CYPMHS) in the community.
We are seeking a highly skilled and experienced Senior Clinical Practitioner to
join the CYPMHS dynamic Transitions Team in Leeds.
We hope to find an exceptional candidate with the right attitude, skills,
knowledge and an enthusiasm for working with young people and families.
The transition journey begins aged 17 and can be complex to navigate. You must
have a sound knowledge of mental health issues in adolescence and treatment
pathways alongside robust risk management skills and an understanding of the
complexities and needs of service users who are transitioning to Adult
Services, either CMHT or voluntary/3rd sector.
Our aim is to ensure that young people transitioning have the opportunity to
develop a relationship with a professional from adult services prior to final
transfer and are both supported and engaged in the process.
Main duties of the job
Effectively
and safely manage a small but complex caseload of young people aged 17 who
require transitioning out of CYPMHS. Making referrals, consulting, formulating,
joint working and offering consultation on other cases.
Participate in the development of the interface between adult and CYPMHS.
This
will include supporting young people to access non-NHS community provision
where appropriate. Establish and maintain an up to date, enhanced awareness of
suitable ongoing mental health and wellbeing services that can support young
people post 18. Develop and maintain effective working relationships with these
services including voluntary sector, community services and with Social Care to
benefit young people.
Clinically lead the team and support junior colleagues, offer clinical and
caseload management supervision and mentorship. Be an educator, facilitate the
sharing and development of skills and knowledge within the team. Support the
team manager in the line management of the team.

Job Purpose
The post holder will be
responsible for delivering and leading highly specialist clinical care to their
own caseload and service within their speciality area. In doing so, they will
assess, plan, deliver and evaluate patient centred programmes of care ensuring that
they practice within their sphere of competence and knowledge. The post holder
will work to professional and regulatory body codes, standards and guidance at
all times ensuring that their practice is grounded in evidence based
theoretical and practical knowledge. They will be responsible for developing
new and innovative clinical practices responding to emerging knowledge and techniques
