Job summary

Our staff led the way in moving from the title of Community Nursery Nurse to this new role to reflect the changing dynamic of our work with families across the city. Your role will be to assist in the delivery of the Healthy Child Programme, offering support and interventions to children and their families. You will be actively involved in seeking out health needs through detailed assessments, therefore you will need to have the skills and knowledge to identify and respond to these.

Awareness of safeguarding issues is crucial alongside the ability to identify and appropriately respond to concerns. We work using a partnership approach with the multi-agency team and wider agencies therefore good verbal and non-verbal skills are essential in ensuring local health needs and priorities are addressed.

Main duties of the job

Alongside 0-19 colleagues and partner agencies you will demonstrate effective team working and communication skills in supporting families through timely interventions, to understand key developmental milestones, promote healthy lifestyles and school readiness. A typical working week could include: completing development reviews in clinics, Childrens Centres and the family home; facilitating breastfeeding groups and individual one to one support; offering face to face and virtual support to families via virtual platforms and in our integrated Baby Hubs across the city; working with our key partners to deliver the HENRY programme; supporting parents through our evidence-based pathways with positive parenting interventions such as toileting, nutrition, managing child behaviour/ sleep, speech and language and overall child development; signposting to other key professionals and completing referrals as appropriate. Job responsibilities

Job Purpose

The post holder will work within standard operating procedures / pathways to manage their own workload independently but with reference to a supervisor within the clinical area. In doing so, they will support the assessment of patients and implementation of programmes of care ensuring that they practice within their sphere of competence and knowledge. The post holder will work within a range of standard operating procedures and pathways in which they have received training in order to be competent.

Key Responsibilities

1. Clinical

1.1. The post holder will work within agreed framework to plan, implement and evaluate clinical and/or therapeutic activities and patient response to activity.

1.2. Support and assist other staff members to deliver care as delegated and directed

1.3. Demonstrates highly developed physical skills obtained through practice and training e.g. accuracy, dexterity and coordination to complete tests and assessments within agreed pathways of care, manipulate equipment.

1.4. The post holder will be able to make judgement involving facts or situation which may require analysis e.g. alter programmes of activities or making clinical decisions related to specific pathway of care.

1.5. Able to identify risk or changes in condition and is responsible for taking action; advises supervising professional and accurately record all interventions

1.6. Provides and receives routine or sometimes complex information using persuasion, reassurance, tact and empathy and addressing barriers to understanding in order to engage the individual in their care and gain valid consent for care; record discussions in an accurate and timely manner in the individuals patient record

1.7. Approaches each individual with care, compassion and sensitivity ensuring that these values are reflected in all aspects of their role

1.8. Engages and actively involves the individual and, if appropriate, their family/carer, in their assessment, programme of treatment and/or care by using person-centred techniques to promote a culture of self care, involvement and empowerment

1.9. Prioritises their own workload and understands the boundaries of their practice; may plan tasks for others e.g. patients, the workload of more junior staff

1.10. Be responsible and accountable for clinical administration tasks and management of resources within the clinical area e.g. stock control, basic equipment cleaning, specific clinical tasks to support the smooth running of a clinic.
