Job summary
MindMate Single Point of Access are looking for clinicians
who are passionate about children and young peoples emotional wellbeing
and mental health to join our team. An ideal opportunity for
practitioners looking to develop their experience and knowledge as a band
6. We will also consider applications from newly qualified practitioners
to join us as a band 5 supported by preceptorship & training to
develop your skills.
We receive referrals from young people, parents/carers, and professionals
for children and young people of school age who need support with their
emotional wellbeing/mental health. This also includes neurodevelopmental
referrals for autism and ADHD. Referrals are screened and triaged in
order to refer them to the most appropriate service.
A multiagency team,
working in collaboration with other agencies including CAMHS, Northpoint,
The Marketplace and Clusters. We offer full/part-time hours with hybrid
model of working, with both office and remote working opportunities
Main duties of the job
An ideal opportunity to gain a broader
understanding of children and young peoples mental health in Leeds,
including specialist mental health services and to develop skills in this
area. Working in a multidisciplinary team you will be responsible for
receiving and triaging referrals to identify the most appropriate service for
children and young people.
This includes speaking to the young person/family,
contacting the referrer, gathering information from professionals, clusters
and schools to support the triage. Triage for more complex referrals may
include a face to face consultation with the young person / family.
Formulation panels and consultation with professionals within CAMHS will
support clinical decision making and full multi-disciplinary support is
readily available. You will have excellent assessment and communication
skills and thrive working in a fast-paced environment, supported by a wealth
of experienced clinicians and a supportive leadership team.

for their own caseload within their speciality area as an autonomous
practitioner. In doing so, they will assess, plan, deliver and evaluate patient
centred programmes of care ensuring that they practice within their sphere of
competence and knowledge. The post holder will work to professional and
regulatory body codes, standards and guidance at all times ensuring that their
practice is grounded in evidence based theoretical and practical knowledge. The
post holder will provide the day to day line management of a team of
clinicians.

Maintain clinical credibility demonstrated through
undertaking clinical practice at a frequency agreed with Service
Manager/Team Manager/Senior Practitioner Pathway Lead in the relevant field.

Practices as an autonomous clinician to support
service delivery, responding to social, scientific, clinical and ethical issues
in line with the values and agreed model within the service

Works to standards of proficiency identified by
their professional and regulatory bodies

To
comply with the organisations Infection Prevention and Control requirements,
including bare below the elbows dress code for staff with a direct care
delivery role
