Job summary
Camhs services in Leeds are commissioned to work with young people up until their 18th birthday. The transitions team is set up, equipped with specialist knowledge about what services and opportunities are available for young people to provide an appropriate level of support post 18. Camhs clinicians refer young people they are working with for a transitions consultation and possible intervention which will be a joint approach to agreeing post discharge support packages. We are looking for a candidate to clinically and operationally lead the transitions team.
Main duties of the job
The post holder will be responsible for the line management and clinical leadership of a small multidisciplinary team focussing on working with 17 and a half year olds - an interesting and autonomous mix of working clinically with young people and families, working collaboratively with camhs colleagues to navigate the most successful routes for young people out of camhs and into adult services. Its a great opportunity to work with all pathways within camhs and to network with other services, keeping up to date with multi-agency citywide developments.
