Job summary
The
Community Discharge Assessment Team (CDAT) aims to prevent avoidable hospital
admissions and works closely with the multi-disciplinary team, providing
expertise in discharge planning and ensuring a seamless transition between
hospital and community services. CDAT Discharge Facilitators are a mixture of
Registered Nurses, Physiotherapists and Occupational Therapists.
The service aims to promote a good discharge experience for patients and their
families/friends. Our main areas of work are the ED/SDEC (Same Day Emergency
Care) and the acute assessment wards while working in collaboration with our
community services.
CDAT holistically assess patients and identify opportunities for timely
discharge where the patient can be safely managed in the community and avoid
unnecessary admission to hospital. The team works together with community and
hospital colleagues, Adult Social Care and the third sector to deliver best
outcomes for people
Main duties of the job
The
Discharge Facilitators role is to support the transfers of care for patients in
ED/SDEC (Same Day Emergency Care) and the 4 admissions wards. The aim is to
avoid hospital admission or when it has been necessary for a patient to be
admitted expediate discharge.
It is a team of registered Nurses,
Physiotherapists and Occupational Therapists, who use their skills to assess
patients needs and work in collaboration with our community colleagues to
achieve positive outcomes for the patient.
The service is constantly striving
to enhance its offer to improve patient experience. Currently the service is
utilising the skills and expertise of the OT members of the team to provide OT
input which involves ward based assessments and home visits.
The Discharge
Facilitators work 8-8 Mon Fri and 8-6 Sat/Sun and Bank Holidays 7 days a week
365 days a year.

The post holder will be responsible
for their own caseload within their speciality area as an autonomous
practitioner. In doing so, they will assess, plan, deliver and evaluate patient
centred programmes of care to a specialist level ensuring that they practice
within their sphere of competence and knowledge. The post holder will work to
professional and regulatory body codes, standards and guidance at all times
ensuring that their practice is grounded in evidence based theoretical and practical
knowledge.
