Job summary

An exciting opportunity has arisen to join a dynamic and motivated senior leadership team within LCH NHS Trust - Adult Business Unit (ABU). This role is the Clinical Head of Portfolio for Patient Flow Services. We are looking for a Registered Nurse, AHP or Social Worker with significant experience of leading community services and a proven track record on leading on improving quality and service development. This exciting role will work closely with the Operational Head of Portfolio and in partnership with senior and local leadership teams. The Patient Flow Portfolio is made up of several teams including the Transfer of Care Hub and the Community Discharge Assessment Team both being based in St James Hospital, the Health Case Management Team, Bed Bureau and Recovery Hub Bed Base including Wharfedale Hospital approx 150 beds.

Main duties of the job

You will lead a team that includes registered nurses, AHPs, Social Workers and Pharmacists. As a senior clinical leader you will be supported by the local operational and clinical leaders across the portfolio of services. You will provide line management for people in clinical leadership roles within these teams. LCH NHS Trust is forward thinking organisation and we are leading work to modernise Intermediate Care and Urgent Community Response, working closely with system partners across Leeds, to enable effective care delivery. We are also committed to develop you as a clinical leader and provide a supportive learning environment. The Trust is a teaching organisation and provides placements, for undergraduate and postgraduate nurses, AHPs, Psychologists and have nursing & AHP students working in LCH whilst undertaking apprenticeship training.

Job responsibilities

The post holder will be responsible for the clinical delivery of services and pathways over a defined specialist area of the organisation. Working closely with the Business Unit Clinical Lead, Professional Leads and Medical leads, the post holder will provide clinical leadership and direction to the identified service area supporting the integration and transformation of service delivery.

They will ensure that high quality and safe services are delivered efficiently and effectively. The post holder will work to professional, operational and regulatory body codes, standards and guidance at all times ensuring that practice is grounded in evidence based theoretical and practical knowledge. They will be responsible for developing new and innovative practices and systems responding to emerging knowledge and techniques by interpreting and implementing national and local drivers.
