Job summary
A fantastic, exciting, and challenging opportunity has arisen in the Health Case Management Service. We are looking for a dynamic, self-motivated Band 6 Health Case Manager (HCM) to join our team. We welcome applications from health professionals from a wide range of backgrounds including Nurses, Allied Health Professionals and Social Workers. It would be essential for applicants to have a professional registration and a Best Interest Assessor (BIA) qualification or have the aspiration to undertake the BIA training. You will have experience of working with adults and their carers within a hospital and /or community setting. You will have excellent organisational skills and be able to work on your own independently and creatively using a problem-solving approach, as well as part of a busy team. It is essential that you are motivated, organised, a team player and enjoy the buzz of working in a fast paced and dynamic environment.
Main duties of the job
The Health Case Management Service provides a case management function for the population of Leeds (18 years and above) who are eligible for NHS Fast Track and Continuing Healthcare funding; this includes hospital and within their own home, over 7 days, Monday to Friday 08:00 to 20:00 and Saturday and Sunday 08:00 to 18:00. The post holder will - Manage a complex caseload. Work alongside MDT including, ICB, Neighbourhood Team, LTHT and legal team. Support with coordinating of day-to-day tasks and referrals. Supervise and mentor staff Have opportunities to be develop skills in safeguarding, personal health budgets, clinical supervisions, deprivation of liberty, mental capacity, clinical nurse assessments for end-of-life pathways. The post will initially be based at Killingbeck Court, Killingbeck; however, the service is citywide, and staff may be required to work flexibly across the city in the future.
