Clinical Nurse Specialist - Stoma Care

Job summary

An exciting opportunity has arisen to join our Stoma Team. We are looking for someone who is passionate about caring for stoma patients from discharge from hospital to their home, teaching patients how to care for their stoma as well as providing psychological and emotional support. The successful candidate will lead the stoma team, working closely with the Acute Stoma Care team, GPs and community neighbourhood teams and will form part of the Continence Urology and Colorectal Service (CUCS). As well as providing stoma skills teaching to small groups of community health care professionals, the successful candidate will also provide stoma care education to our key stakeholders. To achieve this, you will need to be highly self-motivated and have excellent communication and interpersonal skills. You will also have to have the ability and confidence to work autonomously, under pressure and often alone and adhere to the safe lone worker policy.

Main duties of the job

The successful candidate will aim to see new patients within 3-5 days of discharge from hospital in their own home. They will provide teaching to the patient to aim for independence with stoma care, providing diet and fluid advice, emotional and psychological support. You will be expected to have Leadership and Management skills as a key part of your role will be to manage the team of three stoma nurses, carrying out their Appraisals, 1-1s and team meetings. You will be expected to develop the service to ensure high standards of stoma care at all times. You will also provide education to GPs, Community Neighbourhood Teams and Care Homes. You will also provide Clinical Supervision to the team and will be expected to maintain up to date with all stoma products by seeing stoma representatives regularly. You will be supported by the Continence, Urology & Colorectal Service Clinical Lead. We look forward to a successful applicant joining out team.

The post holder will be responsible for delivering and leading highly specialist clinical care to their own caseload and service within their speciality area. In doing so, they will assess, plan, deliver and evaluate patient centred programmes of care ensuring that they practice within their sphere of competence and knowledge. The post holder will work to professional and regulatory body codes, standards and guidance at all times ensuring that their practice is grounded in evidence based theoretical and practical knowledge. They will be responsible for developing new and innovative clinical practices responding to emerging knowledge and techniques and working with other services where appropriate to make improvements.
