Are

in the Community Care Beds could be the one for you. Home first is the ideal
outcome for many patients leaving hospital and providing more care in persons
own home is preferred, but a temporary stay in a setting that has a homely
environment and that can provide 24-hour care is sometimes required. The
Community care beds do this, enabling early discharge from hospital and
reducing hospital admissions. The CCB service delivers complex rehabilitative
care of the frailer older adults in Leeds and the nursing care is delivered
alongside a wider multi-disciplinary team. The focus always is on the
individual, helping them progress to going home. The CCB service offer flexible
work patterns.
Main duties of the job
Community
nursing in the care beds suits people who can work independently and within the
wider multi-disciplinary team. Youll be resilient, motivated, and adaptable to
working in an ever-changing environment and of course, a passion for providing
the best possible care to all. You will have great communication skills and
maintain a high standard of clinical skills when carrying out nursing
procedures. You will ensure compliance with our clinical governance framework,
you will motivate colleagues, champion service user safety and dignity and
drive continuous improvement.
As a Senior Nurse you will provide leadership to the staff nurses, care staff
and report directly to the Clinical Quality Lead Nurse.

Job responsibilities
The post holder will be responsible for the caseload
within the recovery hub and act as senior clinician on duty. In doing
so, they will assess, plan, deliver and evaluate patient centred programmes
of care to a specialist level ensuring that they practice within their
sphere of competence and knowledge. You will act as the senior nurse
for a group of patients and ensure that they are effectively managed by
having regular discussions and oversight from band 7 clinical leads
within the team and conduct regular joint reviews with the wider health
and social care team. You will holistically assess and review the needs
of patients in collaboration with the multi-disciplinary team members
to ensure all identified needs are addressed, Plan patient contacts and
maintain the nurse diary in order to effectively manage the workload
for safe patient care, making every contact count, Actively lead on
daily handover meetings and safety huddles to plan and evaluate patient
care, Refer and present patient cases requiring a multi-disciplinary
approach to the weekly case management meeting and work with members of
the team to plan and deliver future care, Manage the safe transfer of
care between care providers and make appropriate referrals to other
care providers outside of the integrated team, Work in accordance with
the Nursing and Midwifery Council (NMC) Code of Conduct, Maintain and
lead on documentation standards, whether written or electronic in
