We are looking for motivated District Nurses who are looking for a new challenge and have a clear passion for community nursing.

We are a forward-thinking community organisation delivering care to the adult population of Leeds within fully integrated Neighbourhood Teams.

We are in the incredibly privileged position of caring for people in their home environments and are also developing our integrated community clinic offer including building on the close relationships with the Leeds Primary Care Networks and Adult Social Care partners.

We are recruiting District Nurses in Armley, Pudsey, Seacroft and Middleton Neighbourhood Teams, to provide, clinical care, clinical leadership and oversee the delivery of high-quality care within your Neighbourhood Team.

Due to the nature of the role, it is essential that applicants can travel independently across the locality and have access to a suitable vehicle for business purposes. If necessary, adjustments can be considered in accordance with the Equality Act 2010.

Main duties of the job

Working with our Trainee / Community Matrons you will manage a clinical caseload and lead a caseload cluster of registered and unregistered staff to provide excellent care for our patient population.

Our integrated Neighbourhood Teams offer seamless proactive support and care in the community. The teams are made up of Nursing and AHP- Therapy professionals working closely with our social care partners.

Our service runs from 7am to 10pm every day of the year. The focus at all times is on the individual, helping them remain safely at home, or to come home quickly from hospital.

We are looking for a registered nurse who is dynamic, innovative, passionate and committed with experience of case management and multidisciplinary working. We are looking for clinical leaders who possess excellent communication skills and an ability motivate your colleagues. We require you to demonstrate our trust values and behaviours. We are looking for District Nurses who are resilient and able to support others and our wider leadership team through change in order to maintain the delivery of high quality, compassionate, proactive and safe care.

Job responsibilities

The post holder will be responsible for delivering and leading highly specialist clinical care to their own caseload and service within their speciality area. In doing so, they will assess, plan, deliver and evaluate patient centred programmes of care ensuring that they practice within their sphere of competence and knowledge. The post holder will work to professional and regulatory body codes, standards and guidance at all times ensuring that their practice is grounded in evidence based theoretical and practical knowledge. They will be responsible for developing new and innovative clinical practices responding to emerging knowledge and techniques and working with other services where appropriate to make improvements.

1.1.Utilises highly developed specialised knowledge covering a range of procedures and underpinned by relevant broad based knowledge, experience and competence

1.3.Demonstrates independent practice responding to social, scientific, clinical and ethical issues which are encountered

1.4.Makes complex judgements requiring analysis, interpretation and comparison of options by integrating complex knowledge sources in new and/or unfamiliar contexts

1.6.Engages and actively involves the individual and, if appropriate, their family/carer, in the assessment, planning, implementation and evaluation of programmes of treatment and/or care by using person-centred techniques to promote a culture of self care and empowerment

1.7.Works to standards of proficiency identified by their professional and regulatory bodies performing clinical treatments/procedures to a highly specialist standard

1.8.Prioritises their own workload and that of the wider team within agreed objectives deciding when to refer to others as appropriate.

1.9.Provides and receives complex, sensitive or contentious information where motivational, persuasive, empathetic, negotiating and reassurance skills are required whilst demonstrating an understanding of barriers to communication

1.10. Approaches each individual with care, compassion and sensitivity ensuring that these values are reflected in all aspects of role including the management of complaints and compliments.
