Job summary
We have an exciting opportunity for a motivated diabetes nurse who is enthusiastic about diabetes to join our supportive and dynamic team. We have an opportunity for:
- 1 .x 0.5wte (18.75hrs) band 6 diabetes specialist nurse within the community diabetes service - Fixed term contract for 12 months
- Significant experience of working within type 2 diabetes is essential
- The role offers hybrid working and flexibility in working pattern
- The service is Monday- Friday and covers the entire city of Leeds
- Our service manages predominately type 2 diabetes
- The role involves home visits, clinics, group sessions and some telephone/admin sessions that can be completed remotely.
Main duties of the job
The community diabetes multidisciplinary team consists of specialist nurses, dietitians, GP's and pharmacists, providing care to patients with type 2 diabetes. You will be managing your own caseload and liaising closely with primary and secondary care. We need excellent nursing experience alongside a flair for development within our team. You will have the opportunity to work alongside experiences staff who can help you develop your knowledge, skills and experience. We also offer our permanent staff the opportunity to sign up for the non medical prescribing course.
