Job summary

An exciting opportunity has arisen for enthusiastic and motivated Staff Nurses to join our developing Neighbourhood Response Team at Leeds Community Healthcare. This is a fantastic time to become part of a Response Service that is growing, evolving, and shaping new ways of delivering urgent community care across the city. The Neighbourhood Response Team is a core component of the NHS community services commitment to the NHS 10-year plan and development of Neighbourhood Health. Our Neighbourhood Response Team provides timely, person-centred nursing care within patients homes, supporting early discharge from hospital and preventing unnecessary hospital admissions. We work to clear timeframes of 02 hours, 24 hours and 4-48 hours, ensuring patients receive the right care, at the right time, in the right place. We have bases across Leeds and offer a variety of flexible shift patterns between 7am and 6pm, seven days a week. Main duties of the job

Main Duties of the Role Respond to patients within 02, 24, and 4-48 hour timeframes, ensuring rapid assessment and intervention to avoid unnecessary hospital admissions. Work alongside Community Matrons, District Nurses, Senior Nurses, and Neighbourhood Clinical Assistants. Working in collaboration with many serviced including Neighbourhood Nursing, Neighbourhood Therapy, Neighbourhood Out of Hours and Active Recovery. Collaborate with internal and external partners to co-ordinate safe, effective and holistic care aligned with the Neighbourhood Health model and the National Oversight Framework for community services. Provide high-quality clinical care and carry out nursing procedures in accordance with professional and organisational standards. Deliver person-centred, goal-focused care that promotes independence and self-management. Demonstrate resilience, motivation, adaptability and a passion for delivering excellent care in a fast-paced and ever-evolving environment. Contribute to service development as the Response Team grows, supporting innovation and continuous improvement. Ensure compliance with clinical governance requirements, champion patient safety and dignity, and act as a positive role model to colleagues.
