Falls Specialist Physiotherapist - Community Falls Service Hours - 37.5 hours Band 6 Do you want a role where you can focus on your physiotherapy skills and rehabilitation? An exciting opportunity has arisen for a highly motivated and enthusiastic Band 6 Physiotherapist to work in the Leeds Community Falls Service. The citywide service runs Monday to Friday 8am-4pm. In line with ongoing developments we continue to implement new ways of working and innovative pathways, to support older people maintain their independence and reduce their risk of falling whilst providing care closer to home. This new role will support collaborative working with Active Leeds to provide initial assessments in community settings for their Strength and Balance programme. Main duties of the job

Main duties of the role: As a falls specialist you will work as part of a team in a community setting, alongside other Specialist Physiotherapists, an Advanced Practitioner, Pharmacists and Therapy Assistant Practitioners. There will be an opportunity to develop specialist skills in vestibular assessment and rehabilitation. The team also work closely with Consultant Geriatricians, with twice monthly virtual falls multidisciplinary (MDT) meetings to support management of complex high risk falls patients in the community. You will be responsible for: Providing specialist comprehensive falls assessments and interventions to complex high risk falls patients in the community Providing specialist physiotherapy assessments, and evidence-based and goal-orientated interventions to this client group with highly complex presentations and multi-pathologies Providing initial assessments for people who are referred to the Active Leeds evidence-based Strength and Balance programme Acting as a source of specialist advice, and providing education and training on falls assessment, prevention and management Providing a triage role through the integrated falls referral pathway You will need to demonstrate excellent communication skills and have experience of MDT working with older people. Experience of community working, and falls assessment and management is also essential for this role. Due to the nature of the role, a car driver is essential as the service is a city wide.
