Job summary

Were looking for enthusiastic clinical assistants who want to deliver high quality, patient centred care in the community. We are based in Yeadon Health Centre, but we cover Rawdon, Yeadon, Guiseley, Menton, Pool in Wharfedale, Otley and the surrounding area. Our community nursing teams deliver planned care in patients homes, helping people to remain independent and supported within their local communities. We provide a wide range of clinical interventions, including (but not limited to) Safe administration of medication General wound care, Leg dressings, Bowel care, Personal cares, Self-Management Support. Through our work, we enable early discharge from hospital, reduce avoidable hospital admissions, and promote patient independence and wellbeing at home. Were proud to offer a comprehensive career progression framework that helps you develop your clinical expertise. The service runs 8am to 8pm, 7 days a week, with various shift patterns.

Main duties of the job

As a clinical assistant in our Neighbourhood Teams, youll be part of a compassionate and highly skilled multidisciplinary team, working alongside District Nurses, Community Matrons, Occupational Therapists, Physiotherapists, Neighbourhood Staff Nurses, Therapy Assistant Practitioners, and Self-Management Facilitators to deliver truly holistic, person-centred care. Youll also collaborate closely with our Citywide Services, including the Falls, Continence, Urology and Colorectal Care, and Palliative and End of Life Care teams, as well as GP services, ensuring the best possible coordination and continuity of care for patients across West Leeds. You will need to assess and respond to the changing needs of the patient, working within your own limits and seeking support from the wider clinical team where appropriate. You will provide patients with clinical support for example administering medications, undertaking wound care and carrying out observations. Please note this role does require a full Driving licence and use of your own vehicle which is essential to this job role.
