While increased care at home is often preferred, some patients need a short period of support before they can return safely. Thats where our Recovery Hubs come in. We provide shortterm, rehabilitative care that supports: Early discharge from hospital Reduced hospital admissions Safe, supported transition back home Our teams deliver complex, personcentred rehabilitation for older adults across Leeds, working alongside a dynamic and skilled multidisciplinary team. Main duties of the job

Community nursing within the Recovery Hubs is ideal for someone who can work confidently both independently and as part of a wider multidisciplinary team. You will be resilient, motivated, and adaptable, thriving in a fastpaced and constantly evolving environment. Above all, you will bring a genuine passion for delivering the highest standard of care to every individual. You will have excellent communication skills and consistently demonstrate a high level of clinical competence when carrying out nursing procedures. You will also play an active role in ensuring full compliance with our clinical governance framework, supporting and motivating colleagues, and championing service user safety, dignity, and wellbeing.
