Job summary

Are you a clinician who thrives on clinical decision-making and prioritisation? We are looking for an experienced Triage Clinician to play a pivotal role in our Neighbourhood Team Triage Hub.

We are seeking a skilled and compassionate Triage Clinician to join our Community Nursing team. Our clinical triage role is performed in a high-volume, highly dynamic, and time-critical environment. We are the first step in both planned & unplanned community nursing care where accurate minute-to-minute assessment and prioritisation of patient acuity is essential to.

This role is central to ensuring patients receive the right care, at the right time, in the right place, supporting safe and effective management of referrals and urgent community nursing needs.

You will provide clinical triage and advice for patients in the community, working closely with Neighbourhood Team Colleagues, Acute Trusts, GPs, multidisciplinary teams, and other partner services. This is a varied and rewarding role combining clinical expertise with strong decision-making and communication skills.

Main duties of the job

Key Responsibilities:

  • Clinically triage referrals to community nursing services, prioritising according to clinical need and risk.
  • Provide timely clinical advice, signposting, and safety-netting to patients, carers, and professionals.
  • Allocate and coordinate workloads within the community nursing team.
  • Escalate concerns appropriately and support urgent or complex cases.
  • Maintain accurate and contemporaneous clinical records.
  • Work collaboratively within the multidisciplinary team to support seamless patient care.
