Job summary
Are you a passionate nurse with experience in continence care? We are looking for a dynamic and motivated Band 6 Nurse to join our specialist Continence Service, delivering high quality care across the Leeds community. This role offers the opportunity to work with a diverse caseload, supporting adults with a wide range of continence needs in clinic settings, community locations, and patients homes.
You will play a key part in providing specialist assessments, developing personalised management plans, and delivering evidence based interventions that promote independence, dignity, and improved quality of life. Your work will involve close collaboration with multidisciplinary teams, as well as autonomous clinical decision making, giving you the chance to develop advanced skills and contribute to service development. You'll also support education for patients, carers, and other professionals, helping to strengthen continence awareness across the system.
This is a city-wide service which operates Monday to Friday (excluding Bank Holidays).
Due to the nature of the role, it is essential that applicants can travel independently across the locality and have access to a suitable vehicle for business purposes. If necessary, adjustments can be considered in accordance with the Equality Act 2010.
Main duties of the job
You will provide specialist continence assessments and develop personalised care plans for adults across the Leeds community. You will run nurse led clinics, carry out home visits, and support patients with a wide range of bladder and bowel conditions. You will use your advanced clinical skills to deliver evidence based interventions, advise on treatment options, and promote self management strategies that help patients maintain dignity, independence, and quality of life.
You will manage a varied caseload, making autonomous clinical decisions while working closely with colleagues across the multidisciplinary team.The role includes triaging referrals, prioritising care based on clinical need, and ensuring seamless coordination across community pathways. You will contribute to service improvement, maintain high standards of documentation, and support the development of best practice within the team.
Education is also a key part of the role. You will provide guidance to patients, carers, and other professionals, helping to build confidence and understanding around continence management. Your ability to communicate sensitively, work flexibly, and adapt to the needs of individuals will be essential to delivering high quality, person centred care.
