Job summary

We currently have 2 vacancies within the Health Case Management Service. 1 x Permanent Vacancy 37.5 hours & 1 Fixed Term Contract for 12 Months. The Health Case Management Service provides a case management function for the population of Leeds (18 years and above) who are eligible for NHS Fast Track and Continuing Healthcare funding. Patients can be in the community or requiring support from acute settings. The service operates 365 days a year, over 7 days, Monday to Friday 08:00 to 20:00 and Saturday and Sunday 08:00 to 18:00, Bank Holiday hours 08:00-18:00. A fantastic and challenging opportunity has arisen in the Health Case Management Service. We are looking for a dynamic, self-motivated Band 5 Health Case Manager Coordinator (HCMC) to join our team. We welcome applications from health professionals from a wide range of backgrounds. You will have experience of working with adults and their carers within a hospital and /or community setting. You will have excellent organisational skills and be able to work on your own independently and creatively using a problem-solving approach. It is essential that you are motivated, organised, a team player and enjoy the buzz of working in a fast paced and dynamic environment. Main duties of the job

The successful applicant will: - Manage a caseload of patients with complex health needs and at end of life. Complete holistic, person-centred assessments, including supporting carers. Work alongside MDT including, ICB, Neighbourhood Team and LTHT. Lead on duty and coordination work, managing tasks and referrals. Mentoring staff. Make initial enquiries in relation to safeguarding concerns, on behalf of the local authority. A HCMC (Health Case Management Coordinator) is responsible for taking fast track referrals and duty work. This role involves contacting patients or their nominated named representative to discuss the fast-track funding and gather information about the patient and their environment. The HCMC will also liaise closely with professionals and gather a handover. A package of care can be requested, or we can support with finding a nursing home placement. HCMC attend care planning meetings to discuss discharge options including completion of Mental Capacity Assessments. Duty work includes triage of all referrals and allocation to appropriate workers, supporting patients/ families/ professionals with any queries or requests, managing incoming tasks and e-mails. The service operates citywide and is currently based at East Leeds Health Centre.
