Job summary

The Proactive Support Service for Children and Young People is a newly developed service by Leeds Health and Care Partnership. We are looking for an Allied Health Professional (AHP) who is passionate about working with children, young people, families and carers. The AHP will work within a small team of clinicians to ensure young people, and their support systems receive a timely response to meet their needs when there are escalating risks and placements are nearing point of breakdown. The new service will work closely with and be co-located with the Corporate Parenting Team in the Council, working in an integrated way with other health and care services available to our children and young people with escalating needs. There will be a richness of clinical skills and expertise in the team, from a Consultant Psychologist, Nurses and Occupational Therapy. We are wanting forward thinking, innovative practitioners to join this new service, each must have the child and young person, their family and paid carers at the heart of what they do, and they must not be afraid of breaking down barriers, building new processes and ways of working which reflect excellent practice and partnership working across our system.

Main duties of the job

Applications are invited from Qualified Occupational Therapist and Qualified Speech and Language Therapist who have experience of working with Children and Young people, families and care networks to provide support and advice within their field of expertise. You will join a newly formed Proactive Support Service, a multi-disciplinary team which provides professional support and offers collaborative working. You will be welcomed into a service which prioritises reflective practice, clinical supervision and continuing professional development and will be supported by the Clinical Team Manager and the Consultant Psychologist. Main duties will include Work with Young People, families and care networks to provide support and advice within area of expertise. Work closely with colleagues in wider care teams across the partnership Ensuring a close working relationship and effective communication with partner services delivering inpatient services Participate in team meetings, supervision and personal development. You will contribute to the development of the service and implementation of new ways of working. Identifying areas for improvement Take part in audit as required The successful applicant will hold a healthcare registration ( such as HCPC) and will have some understanding and experience of working with Young People and/or people who are experiencing escalating risks and placements are nearing point of breakdown.
