Job summary
We are looking to replace an End of Life Care Facilitator for Care Homes in Leeds, following a recent retirement of the previous post holder.
The End of Life Care Facilitators for Care Homes are senior community nurse practitioners providing end of life education, training and clinical support for staff working in care homes with and without nursing within Leeds.
The team works closely with care home managers and their staff to increase their knowledge, skills and confidence in caring for residents at end of life, embedding best practice and avoiding inappropriate hospital admissions. The role involves delivering direct clinical care alongside care home staff, providing practice based support, advice and staff development and contributing to the delivery of education, training and clinical audit programmes that support the delivery of high quality care.
The postholder will be part of our wider LCH Palliative Care Team which collectively provides 7 day clinical support and advice. We are looking for a person who has experience of delivering palliative and end of life care in the community as part of a wider multidisciplinary team and also has experience of working with a caseload of patients and in supervising other members of staff .
Main duties of the job
The post holder will support the delivery of palliative and end of life care (PEoLC) to residents within Leeds care homes and be responsible for their own practice within their speciality area as an autonomous community nurse practitioner, ensuring that they practice within their sphere of competence and knowledge.
In doing so, they will:
- Assess, plan, deliver and evaluate patient centred programmes of palliative and end of life care as part of a multidisciplinary team.
- Support and develop the PEoLC knowledge, skills, confidence and competence of care home clinicians to assess, plan deliver and evaluate patient centred programmes of PEoLC.
- Support PEoLC quality improvements within care homes.
