Would you like to work in an adult community setting that offers variety, development opportunities, and the chance to work alongside a range of professionals in dynamic, innovative, and supportive teams? #NoTwoDaysTheSame We have an exciting opportunity to join our Neighbourhood Therapy Teams in Leeds as a Band 4 Occupational Therapy Assistant Practitioner, working alongside Physiotherapists and Occupational Therapists to deliver person-centred rehabilitation. We welcome applications from individuals with previous experience in health or social care who have excellent communication, organisational, and problem-solving skills, and who are keen to enhance their knowledge and skills in community rehabilitation. In this role, you will have the opportunity to organise and manage your own caseload, working independently within your scope of practice and using your initiative. Our teams support individuals to manage their health conditions, regain independence with daily activities, and facilitate early discharge from hospital or community-based beds all with the aim of improving health and wellbeing outcomes. This role is based within either the North or South Neighbourhood Teams in Leeds.
Main duties of the job
Key responsibilities are delivering holistic rehabilitation, which includes provision of equipment, teaching and progressing exercise programmes and signposting to community resources to encourage social engagement. You will also be supporting progression of ability to self-manage activities of daily living, including meal preparation, personal care and medications. You will have opportunities to assist with student placements, help develop others, contribute to service improvement projects, and enhance your own skills, including an option to work towards applying for a degree apprenticeship.

The service is provided over core hours of 8am 6pm across 7 days. Weekend and unsocial hours attract enhanced pay.
