Job summary
Exciting Opportunity: SEND Health Coordinator (Full-time/Part-time) We are seeking two qualified, clinically registered professionals to join our developing SEND Health Team as SEND Health Coordinators. This is a fantastic opportunity for experienced nurses or allied health professionals with experience of working with children and young people with Special Educational Needs and Disabilities (SEND). As part of the West Yorkshire Integrated Care Board (WYICB - Leeds Place), you will play a key role in helping us deliver on the statutory responsibilities set out in the Children and Families Act (2014) and the SEND Code of Practice (2014). You will be supporting children and young people aged 0-25 with SEND to ensure they receive the care and support they need to thrive. We welcome both full-time and part-time applicants. In this role, you will support the coordination of high-quality and timely health advice for Education, Health, and Care Plans (EHCPs). You will play a key role in promoting the importance of health within the SEND framework, offering guidance and support for EHC assessments, Annual Reviews, and contributing to multi-agency panels. The successful candidate will collaborate closely with key system partners, including health, education, local authorities, as well as children and young people with SEND and their families/carers.

The post holder will work flexibly to meet the needs of our system.
