Main duties of the job

As a staff nurse delivering high quality care within a Neighbourhood Team you will: -have excellent communication skills and be able to motivate others. -be dynamic, innovative, passionate and committed with experience of case management and multidisciplinary working. - work alongside District Nurses, Community Matrons, Ots and PTs, Neighbourhood Clinical Assistants, Therapy Assistant Practitioners and Self Management Facilitators in one MDT to provide truly holistic care. -be resilient and able to support others through change to maintain the delivery of high quality, compassionate, proactive and safe care. Are you a registered occupational therapist with an interest in Vocational Rehabilitation who is looking for the next challenge?

Do you have a passion for long term conditions? Would you enjoy working with like-minded OTs on a new project about delivering Vocational Rehabilitation? If the answer is yes, then we would love to hear from you!

An exciting opportunity has arisen within the Specialist Business Unit of Leeds Community Health Care NHS Trust. We are looking to recruit an experienced Band 6 Occupational Therapist to join us in our multidisciplinary team. The successful candidate will work as part of a small team to set up, deliver and evaluate the impact of vocational rehabilitation to people experiencing long lasting problems 12 weeks or more after an infection consistent with COVID-19.

The post holder will have the support of the other members of the of the MDT which include physiotherapists, dieticians, therapy assistants, psychology wellbeing practitioners and a consultant in rehabilitation. This is in addition to consultants from other specialties via a weekly MDT meeting.

We are looking for a part-time applicant (30 hours a week) to work on this project. This could be as a secondment (following agreement from your substantive manager) or as a fixed term contract.

Fixed term/Secondment until 31/03/2026.

This role is to provide specialist vocational rehabilitation as part of the overall management of patients referred to the Leeds Long Covid Community Rehabilitation Service. You will support patients who are currently off sick/at risk of going off sick to return to work and/or remain in work in a healthy and sustainable capacity.

Referrals will be passed from clinicians within the multi disciplinary team for intervention to support with vocational rehab. This offer will include 2 new virtual groups: return to work and remain in work, 1:1 consultations and report writing. As a new intervention, the post holder will contribute to the development of the VR groups as well as evaluating the impact they have on the health outcomes of the patients and their work status.

Excellent communication skills are essential to this role as well as sound IT skills as the intervention will be offered virtually.

This is a city-wide service which operates Monday to Friday (excluding Bank Holidays). You must have a valid driving licence and access to an appropriate vehicle.
