Job summary

The Clinical Pharmacy Team are looking for a pharmacist to join them. The team work geographically (North/East Leeds, South Leeds and North/West Leeds (this post) and support three clinical service areas of the Trust: Home Ward (Frailty), the Community Falls Service, and the Home Ward (Respiratory). You will be providing clinical and professional pharmaceutical services, working with the Multi-Disciplinary Teams to promote the optimised, safe and cost-effective use of medicines. The role includes working virtually with clinical colleagues as well as visiting patients in their own homes, liaison with primary care and contributing to patients remaining at home.

Main duties of the job

The Home Ward Frailty provides co-ordinated rapid care to people with moderate to severe frailty in their own homes. Patients are supported by daily interventions such as medication reviews, blood monitoring, clinical observations and review at the consultant led multidisciplinary team meeting. The Home Ward (Respiratory) supports patients stepping down from hospital care. Patients are supported by a twice weekly MDT meeting where all medicines (not just those for the presenting respiratory condition) are optimised. The Community Falls Service aims to reduce the risk of falling and maintain safety and independence. Deprescribing of medicines is undertaken as required. Please see the A Day in the Life of..
