Job summary
We are looking to recruit a registered NMC Nurse, who has experience as a Diabetes Specialist Nurse to join our team at Leeds Community Healthcare NHS Trust who is enthusiastic, highly motivated and forward thinking, to help us drive improvements in service delivery.
This post is permanent, flexible (hybrid working), full time 37.5 hours Monday to Friday service.

The Community Diabetes multi-disciplinary team works across health care settings and patients homes within Leeds and consists of specialist nurses, dietitians, GP's, pharmacists and diabetes facilitators who provide care and treatment to patients living with Type II Diabetes. This role is designed to provide leadership to our diabetes specialist nursing team and to work alongside our clinical lead nurse, clinical lead dietitian and team manager.
All staff are provided with a laptop and smart phone for hybrid working and remote access to SystmOne.

Main duties of the job
Responsibilities:-
- You will be involved in clinical audits, as well as obtaining patient feedback to support with both service development and clinical team support.
- You will be directly involved in pathway development providing ideas and feedback directly to clinical and operation heads of service.
- You will hold a caseload of patients living with complex Type II diabetes. Within this you will utilise highly developed behavioural management techniques with a focus on patient self-management.
- You will attend and help to facilitate relevant Team meetings, and other meetings as necessary to discuss care of patients and service provision.
- You will provide ongoing clinical supervision for all diabetes nurses within the team to ensure treatment delivery adheres to current evidence, local and NICE guidance and the service specification with alignment to the Leeds Diabetes strategy.
- You will play an integral part in developing current pathways as well as service redesign with a view to enhancing the community Diabetes offer in Leeds including liaising with our primary and secondary care partners.
Essential Requirements
- You will hold a driving licence and have access to a car for work purposes.
- You will be a confident communicator with experience communicating effectively with other members of a team using for example SystmOne, outlook, MS teams or other Trust specific communication methods and be IT literate.
