We are also looking to expand out Community Nurse Bank provision. Main duties of the job

As a Band 5 Community Nurse in the beautiful rural locality of South West Shropshire, you'll deliver compassionate, high-quality care to people in their own homes, bringing autonomy, variety, and real purpose to your day.

What You'll Be DoingDelivering holistic nursing care that helps patients stay at home, avoid unnecessary hospital admissions, and achieve safe, timely dischargeWorking as an autonomous practitioner across a diverse and rewarding caseload, providing key clinical interventions.

  • Experience of working in a multi disciplinary / multi agency health/social care environment.
  • Experience and knowledge of basic administration systems including Microsoft office
  • Experience of successful partnership working arrangements with other services
  • Previous community experience

  • Up to date with Mandatory Training
