The postholder will manage all associated administrative functions, maintain accurate system and service reports, and work to maximise flow across the Trust. Operating as a key member of Urgent and Emergency Care Division working close to Virtual Wards, Capacity Hub, Integrated Front Door Team and Urgent Community Response Teams, they will support the delivery of a coordinated and responsive service that improves patient transitions and system flow. The role involves processing referrals using agreed criteria, maintaining an up-to-date caseload on RIO, and working alongside the Clinical Support Co-Ordinator to oversee capacity within the division, ensuring ongoing transition through the services.

The Patient Flow Co-Ordinator will remain responsive to the administrative requirements of the clinical services and will be expected to work autonomously to fulfil the duties of the post.
