Ward Discharge Coordinator at Ludlow and Bishops Castle

We have an exciting opportunity for a Bank Ward Discharge Co-ordinator to join our friendly teams at either Ludlow Community Hospital or Bishops Castle Community Hospital .

The role involves working within the ward environment as part of the multidisciplinary team, to provide a continuous coordinated approach to the safe and timely discharge of patients with simple and complex discharge needs.

Main duties of the job

  • Communicate and liaise with patients, their families and carers and other key external stakeholders who play an integral role in patient discharge.
  • Support the delivery of the SAFER red to green approach to expediting patient care.
  • The role requires a high proportion of autonomous working with in the ward team within the sphere of the role.
