Job summary

The post holder will work to ensure the development and implementation of transformational change programmes, linked to national drives around hospital discharge and the community home first model. The post holder will work closely with the clinical and operational teamsacross health and social care to transform local services across multiple organisations to support the determination of the right mix for 'place-based', delivery models, focusing on Discharge to assess and home first principle and mitigation of risks of the transition.A key responsibility of the role will be the supporting System leaders with development, facilitation, and coordination of the service improvement and Long - Term plan discharge transformation programmes across the local health and social care system using Quality improvement and inclusive approaches.

Main duties of the job

  • Oversee stakeholder relationships to improve and sustain patient discharge processes.
  • Develop and implement an effective Discharge to Assess care model through the integration of therapies.
  • Deliver national policy objectives through local plans across system partnerships.
  • Lead strategic planning, monitoring, and reporting for discharge service improvements.
  • Support discharge strategy priorities by assessing population needs and reviewing services with system leads.
  • Foster a culture of efficient, high-quality service delivery.
  • Ensure the creation and execution of credible cost improvement programs.
  • Manage delegated budgets effectively.
  • Ensure all services comply with regulatory standards (e.g., CQC/NHSI).
  • Establish proactive management systems to minimize reactive decision-making and handle crises effectively.
  • Analyze and present data to guide decision-making and continuous service evaluation.
  • Encourage a solutions-focused mindset to support vulnerable patient groups.
  • Provide innovative leadership to maximize resource benefits for patients.
  • Oversee organizational changes affecting hospital discharges to ensure optimal outcomes.
  • Lead large-scale service developments aligned with clinical sustainability alongside ICS leads.
  • Coordinate workforce planning with multiple organizations and workforce leads.

  • Educated to master's degree level or equivalent
  • In-depth professional knowledge in a number of disciplines e.g. financial management, performance management, information systems Transformational change, staff management acquired through training and experience over extended period
  • Evidence of continuous personal and professional development
  • Formal Management qualification or equivalent operational experience
  • Registered with the NMC (Nursing and Midwifery Council), Health and Care Professions Council (HCPC) or Social Work England (SWE)

Experience

  • Extensive Experience in implementing large scale multi agency programmes of with significant experience of managing people and services in either health or social care setting.
  • Substantial senior management/leadership experience in the NHS, in variety of settings
  • Extensive experience of delivering high quality services within a complex financial setting
  • Evidence of successful management of staff from a wide range of professionals across a spectrum of services and specialties
