An exciting opportunity has arisen to work within a well-established Community Pulmonary Rehabilitation Team here in Shropshire, Telford and Wrekin supporting patients to make positive life choices that enhance their lives and health.

Main duties of the job

As part of the Pulmonary Rehabilitation team, you will be expected to work independently, using your own initiative to support patients in a community setting.

You will work alongside and build relationships with the community respiratory nursing teams, local GP surgeries, secondary care respiratory colleagues and community inter disciplinary teams, delivering high quality care across community settings.

The role will involve providing pulmonary rehabilitation and respiratory physiotherapy to patients in the community in both group and domiciliary settings, triaging new referrals, chest clearance advice, post exacerbation and admission avoidance care and participating in MDT meetings and events.

Applicants should possess an in-depth clinical knowledge of respiratory conditions, which should be supported by recent experience or qualifications. You will be required to demonstrate comprehensive assessment skills, knowledge of evidence based practice, and critical appraisal skills in applying theory to practice. Support towards CPD will be given including attending external courses and conferences as the service allows.

The successful applicant will demonstrate excellent communication skills, autonomous caseload management skills and an ability to work flexibly within a team.

Applicants must be able to meet the travel requirements of the post.
