? What You'll Be Doing:- Delivering evidence-based care across a wide range of clinical needs
- Supporting patients in their journey to independence in their most appropriate place of care
- Receiving specialist training in Continence Care & Assessment
- Collaborating closely with the Continence Nurse to provide timely, targeted care

Main duties of the job
Main Duties of the Job
You will play a vital role in delivering compassionate, safe, and effective care across a variety of settings. Your work will support individuals in maintaining dignity, independence, and wellbeing.
- Deliver Person-Centred Care: Provide high-quality support tailored to individual needs, ensuring privacy, dignity, and safety are upheld at all times.
- Health Monitoring: Continuously assess and monitor the condition and health needs of patients, working collaboratively with the multidisciplinary team (MDT), families, and carers.
- Risk Assessment: Complete nursing risk assessments using recognised tools to inform safe and effective care planning.
- Continence Care: Undertake continence assessments to identify individual needs and contribute to personalised continence management plans, promoting comfort and dignity.
- Care Planning & Delivery: Contribute to the development and implementation of care plans in partnership with Registered Nurses, ensuring interventions are evidence-based and responsive.
- Ongoing Assessment: Recognise when reassessment is needed and refer appropriately to ensure timely action and optimal care outcomes.
- Medication Administration: Once deemed competent, safely administer prescribed medications in line with clinical guidance.
- Prioritisation & Escalation: Identify changing priorities in patient care and seek guidance from Registered Nurses or other healthcare professionals when necessary
