Job summary
This is an exciting opportunity for you to join the South East Community Nursing Team in delivering holistic community-based care, through a proactive approach. Our patients are people with long term conditions (LTC), very complex health needs, Very High Intensity Users (VHIUs) of hospital services, or people acutely unwell who can be safely managed at home. We utilise the virtual ward to enhance the health and wellbeing of patients, prevent avoidable hospital admissions and improve the patient experience.
Applicants must posess the following:
- NMC Registered General Nurse.Registration (Part 1 Adult / General level 1).
- Level 6 Specialist Practitioner Nursing Qualification or equivalent level 6 qualification in health-related subject.
- Level 6 modules in long term conditions, and physical assessment, or willingness to complete these within agreed time frame.
- V300 Non-medical prescribing qualification or a willingness to complete this training within an agreed time frame.
- Post qualification study to master's level or equivalent or willingness to complete this within agreed time frame.
You must be able to meet the travel requirements of the post.

Main duties of the job
The post holder will provide a structured period of intensive intervention using a step up and step-down approach for patients on the community nursing caseload, providing advanced assessment, prescribing, interventions, care planning and facilitate multi-disciplinary working to enhance the persons health and wellbeing.
The role will include the promotion of patient autonomy and activation for self-care / self-management, and forward plan for patients on their caseload to prevent avoidable hospital admissions.The post holder will be the clinical expert for contemporary community based holistic nursing care in the locality and take the lead for developing and improving the community team's competency in delivering advance care planning and health promotion activities, complex care and nursing procedures towards improving the patient's health and wellbeing and quality of life.
The post holder will work closely with the team leader, to provide senior leadership for the team and be responsible for delivering a high quality service.
