Job summary

Would you like to be a crucial part of an innovative project? An exciting opportunity has arisen for Health Care Support Workers to join the Shropshire, Telford & Wrekin Virtual Wards across all the localities.

The Virtual Ward is a system wide initiative, working in partnership to provide our patients with access to rapid assessment, treatment, virtual monitoring and care planning to stabilise and support an acute episode of ill health or a deteriorating chronic condition that would alternatively require admission to hospital.

The service offers an appropriate alternative to admission pathway, which aims to deliver a recommended and agreed medical, nursing & therapeutic treatment plan. The focus is supporting the safe management of people in their own home. The role and service offer integration and partnership working across the system to ensure a seamless service.

Full time and part time positions are available.

Main duties of the job

You will be responsible to support the delivery of quality healthcare interventions to patients living in their own environment in the community, in order to meet their health and wellbeing needs, and to facilitate re-ablement support in order to maximise a patients functional independence at point of referral.

You will support individuals and clinical teams to deliver high quality, clinically effective patient care through assessment, planning, implementation and evaluation. Along the way, you'll build strong relationships with a wide range of healthcare professionals and patients.

  • GCSE Grade A-C in English & Maths or equivalent.
  • NVQ Level 3 in Direct Care or equivalent.
  • Venepuncture Certificate of Competency or willing to work towards.

Experience

  • Demonstrable experience in a healthcare setting.
  • Ability to work with MS Office package or willingness to work towards.

Knowledge and Skills

  • Awareness of long-term conditions.
  • Awareness of needs of palliative care patients.
