Job summary

The post holder will be a member of the Prison Healthcare Team working as a Trainee Advanced Clinical Practitioner. The post holder will be responsible for acquiring the skills at Masters level to be able to work as an autonomous Advanced Clinical Practitioner on completion of the Masters qualification.

The post holder will contribute to the development of the new model of care at HMP/YOI Stoke Heath and will support the Lead GP and wider healthcare team to improve and deliver excellent care to those held in custody.

The development of the role through Masters level study will provide daily advanced clinical expertise and decision- making in clinical care in line with the National Framework for Advanced Clinical practice. Embracing the four pillars of clinical practice; leadership and management, education and research.

To support and through development of new skills provide daily, consistent, high quality, person centred care is delivered to the patients by assessing individual needs, initiating diagnostics, determining differential diagnosis and initiating appropriate holistic, evidence based treatment with the development of a personalised clinical management plan, including independent prescribing.

Main duties of the job

  • Support the Lead GP and wider healthcare team in identifying further developmental needs for the wider team and support them to address these.
  • Through continual development and study be able practice autonomously within professional boundaries and liaise with medical colleagues and the Lead GP to ensure the safe delivery of treatment to patients.
  • Assess and treat from a range of previously undiagnosed differential diagnoses, formulating treatment plans to manage patients in the prison setting with supervision and support from the Lead GP.
  • Respond to actual and potential health problems appropriately including the management of deteriorating patients, Advance Care Planning for patients within the last 12 months of life and care in the last days of life with supervision and support from the Lead GP and wider healthcare team.
  • Through continual development and acquiring advanced practice skills deliver an advanced level of holistic care: utilising support services appropriately.
  • Support the Lead GP and wider healthcare ream to provide clinical leadership and expertise to the team in relation to advanced clinical practice.
  • Support the Lead GP and wider healthcare team in identifying further developmental needs for the team and supporting them to address these.
  • To promote the advancement of clinical practice.
