We are recruiting to a variety of roles working as part of an innovative MDT. You will be working for Shropshire Community Health NHS Trust with your base at either the Royal Shrewsbury Hospital or The Princess Royal Hospital.

As a registered Advanced Clinical Practitioner you will be experienced in working in a multi disciplinary / multi-agency health / social care environment with knowledge of IT basic administration systems including Microsoft Office Suite (Word, Excel etc).
You will be patient focussed and have the ability to carry out comprehensive assessments and be able to prioritise and make sound decisions, sometimes under pressure.
Main duties of the job
The post holder is required to deliver an advanced level of clinical practice autonomously in the field of geriatric medicine, Frailty, Subacute care and End of Life for patients on our Subacute orthopaedic, frailty and stroke pathways.
The role will ensure that consistent, high quality, person-centered care is provided daily through applying advanced level clinical judgments and critical thinking to assess, investigate, diagnose, prescribe, plan, implement and evaluate clinical care in line with the National Framework for Advanced Clinical Practice. Embracing the four pillars of clinical practice; leadership and management, education and research.
The post holder will support patients who's care is being managed in the community on the STW Virtual Ward, by completing enhanced assessments and treatment planning in line with advanced competencies.
