An exciting opportunity has arisen to become part our large pharmacy team at Shrewsbury PCN. We are actively recruiting with our partner Shropshire Community Trust, where the Medicines Management Team has a long-standing history of delivering high quality services to support the community. This is a permanent role with clinical and leadership opportunities.
At Shrewsbury PCN we have an established clinical team to help to deliver the Direct Enhanced Service (DES) for our patients. Our PCN is made up of 15 practices and you will be allocated dedicated practices, in addition, you will also spend time working at our dedicated PCN hub (Morgan Place) in Shrewsbury business park and local care homes within the network.
You will be supported with your training and developmental needs. The Trust and PCN will provide:
- one to one mentorship with a dedicated Lead Pharmacist during induction phase
- For band 6 applicants, we will develop you towards reaching band 7 level.
- regular team meetings focussing on group-based learning to support clinical knowledge
- a senior management team with a clinical background
- peer mentorship from our Principal Pharmacist
- regular access to clinical supervision
- access to CPPE Primary Care Pharmacy Education Pathway (if not already completed)
- Option to undertake independent prescribing qualification as part of role development
Main duties of the job
You will work alongside other band 6 / 7 clinical pharmacists and pharmacy technicians. Clinical supervision and role development will be provided by a lead pharmacist along with the Trust's Principal Pharmacist.
You will be a key member of the practice teams and an integral part of the team working across the network. Your clinical work in the practices and care homes will involve face to face consultation with patients in managing their long-term conditions.
Your clinical work will involve the management of long-term conditions, structured medication reviews, deprescribing and supporting staff with medication queries. In addition to this, we are also undertaking some exciting work with our patients that take high dose opiates or gabapentoids for chronic pain to ensure dosing is safe as per NHS England guidance to reduce mortality due to the overuse of these agents.
You will be a clinical pharmacist or pharmacist wishing to develop themself. Once embedded into the team and having reached band 7 level, you will be provided with opportunities to support the training of the Foundation Pharmacists we host as part of a partnership with our local Acute Trust as well as pre-registration pharmacy technicians (SCHT levy) and therefore, you will be able to hone your mentoring skills within this role. You will be a key member of the practice teams and an integral part of the team working across the network.
