Job summary

The Community Neuro Rehabilitation Team (CNRT) is a multidisciplinary team providing services across the county of Shropshire, including Telford & Wrekin, for people with acquired brain injuries and progressive neurological conditions. The team comprises of registered Physiotherapy, Occupational Therapy, Speech and Language Therapy and Psychology staff in addition to our Support Workers.
The successful applicant will demonstrate excellent communication skills, an ability to work flexibly within the team and be an open and positive individual who wishes to strive to see the best for both our patients and staff.

Main duties of the job
You will be involved in providing and evaluating goal focused rehabilitation and reablement programs for patients with a range of acquired neurological conditions.
You will work as part of the multidisciplinary team in providing therapeutic goal-based programmes to service users both in their homes and at SCHT bases. This will involve both individual treatment sessions and group activities.
You will undertake delegated tasks, which, for example may include practice of functional activities, transfer and mobility practice, cognitive and perceptual work and provision of equipment for patient use.
You will use effective communication tools, verbal and non-verbal, to communicate treatment programmes to patients who may have barriers to understanding or communicating e.g. deaf, blind, dysphasic or who do not have English as their first language.
You may be required to deliver and fit equipment in patient's home as directed by a qualified member of the team.
