Job summary
In April 2019 we commenced a new contract commissioned by NHS England and led by Shropshire Community Health NHS Trust in partnership with other providers, to deliver care to prisoners based at HMPYOI Stoke Heath. We provide services for 750+ gentlemen in our care with diverse and often complex needs.
As part of a primary care team, you will be experienced in delivering general adult health services to our patient group. Our service incorporates partnership working with mental health and substance misuse services, to ensure that every patient has equal access to services and coordinated care packages.
The post will allow for an experienced adult nurse to develop skills and knowledge within the nursing sister post, supporting the clinical nurse manager with the delivery, monitoring and improvements of general health services. Also, to line manage other members of the team and improve relationships within the multi-disciplinary team. The posts can be both physically and emotionally demanding. You must be able to demonstrate a positive non-judgemental approach to your work and a willingness to work hard in this challenging environment to achieve the best outcomes for patients.
Main duties of the job
To provide a primary health service to patients/prisoners within HMP/YOI Stoke Heath.
To comply with the prison security requirements, and operational policies and procedures.
To provide a high standard of care and promote a good image of the service.
To act as the duty manager for all providers in relation to daily operational issues i.e. staffing, resources, incident management.
To be line manager to a team of primary health nurses.
To support our partner organisations by working in an integrated manner to ensure administration services are provided across Stoke Heath Integrated Care.
