Job responsibilities

Oversee and plan primary health services to ensure that identified patients receive Assessment, planning, implementation and evaluation of mental health needs.

Support patients and exercise positive Use of statutory and organisational requirements and guidelines, relating to the administration of medicine.

Respect patients individuality and maintain their privacy, dignity and human rights within the constraints of the custodial environment.

Support patients to address issues which effect their own health, behaviour and social welfare. Provide patients with support during healthcare activities.

Support patients who are distressed who could be at risk of harm to themselves or others, or who are at risk of suicide.

Take responsibility for clinical treatment or appropriate referral to doctors or outside agencies, also removal of risk factors in a situation where risk of self harm, or harm to others or suicide seems evident.

Develop relationships with professional boundaries as part of a purposeful caring regime.

Respond to healthcare and prison emergencies, relating to prisoners, staff and visitors, within the establishment in accordance with policies and procedures.

Communicate effectively with other members of the healthcare team and other disciplines as required.

Maintain individual responsibility for accurate healthcare records, participate in clinical supervision and in all aspects of clinical governance in accordance with NMC guidelines.

Support the wider team by undertaking triage to determine clinical priorities and need for relevant referrals and to recognise and treat minor illnesses appropriately and independently.

Deliver evidence based healthcare according to the care plan and treatment regimes.

Administer medication as prescribed according to NMC guidelines.

Administer over the counter and other medication in accordance with national instructions and local protocols. Administer medication under patient group directions in accordance with local policy and protocols.

Use health promotion strategies to improve patients health and social wellbeing and aid disease prevention. Where appropriate deliver nurse led health promotion clinics.

Use knowledge, expertise, skills and risk assessment tools to make clinical decisions, and to inform the decisions of other healthcare professionals.

Assess, plan and implement and evaluate care plans and treatment regimes based on patient need and evidence based practice.

Act as a resource for other healthcare professionals and prison staff and provide teaching and mentoring to students and other healthcare staff. To supervise students and junior staff.

Liaise with prison officers, and with patient consent, external healthcare providers, regarding the admission and discharge pathway of patients.

Demonstrate and apply knowledge of the Mental Health Act, common mental health problems and possible management approaches.

Recognise the signs and symptoms of drug and alcohol withdrawal and of the patients need to manage these effects.

To run nurse led intervention clinics following relevant specialist training.

To liaise with internal and external health services. To oversee the transfers of acutely mentally ill patients.

To work closely with our partner organisations to ensure clinical mental health pathways are followed by all team members, and that shared care patients between primary and secondary mental health services are regularly monitored.

To use appropriate clinical IT systems to record care delivered.

To provide clinical supervision to primary health nurses in relation to caseload and care coordination.

To support the clinical services manager in the delivery of all primary care services, governance, and improving quality of care for all.

Responsible for the direct line management of the primary health nursing team.

May be exposed to distressing information and / or incidents.

The post-holder will be required to offer advise, support and assistance to new members of staff acting as a mentor, providing training and advice as appropriate.

Work with senior managers to ensure service users are involved as much as possible in decisions about the service.

Meeting patients nursing care needs as identified, or where care has been transferred to the post holder by the GP. This will usually be in surgery, but may require in cell visits at times.

Maintain through provision as required health screening and health promotion clinics for areas identified through the Health and Justice performance indicators, QOF registers, quality dashboard.

To complete monthly audits on performance data, plan and schedule clinics to maintain compliance with performance indicators with a monthly report to be submitted to the clinical services manager on compliance.

To act as lead nurse for primary health services, with close liaison with other health providers.

To assist in the identification of complex patients requiring case management at a high level.

To participate in care coordination of patients with complex care needs.

Participate in working in collaboration with our partner organisations to improve the quality of care by providing enhanced clinical assessment and intervention.

Assist in supporting the nursing team and our partners in developing crisis management plans and delivery of complex care, based on full assessment of mental health needs for those patients deemed high risk of self harm and attempted / completed suicide,

Participate in providing support with the implementation of evidence based clinical policies to underpin effective and consistent case management.

Participate in identifying areas (self or team) for skill / knowledge development and apply these to practise to provide continuity and high quality patient centred health interventions.

Use a high level of interpersonal, IT and communication skills to communicate effectively with patients and staff groups, including communication of complex and / or sensitive information.

Maintain a high level of performance and be goal and outcome focused when faced with opposition or when working under conditions of pressure.

Where trained and competent to do so, use non-medical prescribing to reduce referrals to GP services for patients supported by primary mental health services.

To provide support and assistance to colleagues during emergency code calls within the establishment.

To administer medications on residential wings, or within the healthcare centre when able to, to support the team.

Training and Qualifications

Essential

• Registered General Nurse - Level 1
• Diploma / Degree in nursing studies
• Non-medical prescriber (V300) qualification, or willingness to attain this
• Evidence of appropriate knowledge base in primary health
• Competency in risk assessment for primary health
• Experience and knowledge of confidentiality and data protection act.

Experience

• Experience of working on own initiative and organising own workload without supervision
• Experience of working to tight and often changing timescales
• Experience of working as part of a team
• Experience of line manager duties / managing others within a team, or willingness to gain these skills
• Demonstrate an understanding of audit
