Job summary

Community Neuro Rehabilitation Team (CNRT) Physios work as part of the wider Interdisciplinary Team, providing high-quality neurorehabilitation to the population of Kensington and Chelsea

The post holder will support the rehabilitation of adults living with acquired and progressive neurological conditions, helping to optimise their independence, health, and wellbeing, and to prevent hospital admission. This will include working with patients on both the Stroke ESD and Specialist Neurological Outreach Service (SNROS) pathways.

This is a key role within the team, working with patients, service users, & carers, using patient centred, goal focused approach, providing highly specialist Physiotherapy assessments & treatment plans. Supporting individuals to maintain or regain mobility, independence, and wellbeing within their own homes and the wider community. This will include in-reaching into local nursing homes.

The post holder will provide clinical leadership within the locality, offering advice to Physiotherapists and the wider MDT. They will be responsible for the education and supervision of junior Physiotherapy staff, Assistant Practitioners, Rehabilitation Assistants, and Physiotherapy students. The post holder will be supported in this role by the locality Team Lead and Tri-borough Clinical Lead Physiotherapist

Main duties of the job

  • Providing a highly specialist Physiotherapy service as part of a multi-disciplinary team, to patients in their own homes, the wider community, and within a clinic setting.
  • To provide clinically effective, person centred, and evidence-based Physiotherapy assessment and intervention to patients with complex physical health needs
  • To assist the Team Lead and Clinical Lead in the planning and delivery of Physiotherapy services to the patients, their family/carers, students, Apprentices complying with Health and Care Professions Council (HCPC) and Chartered Society of Physiotherapy (CSP) professional and service standards of practice.
  • To provide support and supervision for junior staff and students.
  • To act as a practice educator for students on their placement and support junior staff members and colleagues in doing so.
  • To assist in the management, development and research of Physiotherapy services, including service reviews. To hold responsibility for defined projects.
  • To provide support and training to all staff with regards to Physiotherapy interventions.
  • To comply with the HCPC and CSP Code of Ethics and Professional Conduct
  • To deputise and lead the service as and when needed in absence of Clinical lead/ Team Lead
  • To participate clinical triage with other senior therapists
  • To work flexibly

Experience

Essential

  • Post registration experience at band 6 level or above in a wide range of clinical areas i.e. elderly rehabilitation, community rehabilitation and orthopaedics etc.
  • Experience of working in a falls and Rehabilitation team or a sound knowledge of falls and Rehabilitation
  • Experience of working in MDT and leading Multidisciplinary meetings
  • Postgraduate experience of working in a multi professional team
  • Experience in teaching and training and facilitating Groups therapy sessions.
  • Experience of assessing home environment and making onward referral to appropriate members of MDT within and across services
  • Experience of completing Holistic assessments, interventions and care planning.
  • Experience of completing triage
