An exciting opportunity has arisen for a motivated and enthusiastic Occupational Therapist to join the Hospital at Home Therapy Service within Central London Community Healthcare NHS Trust in Barnet.
Our innovative Hospital at Home service supports adults with acute and complex health needs across Hospital at Home, Frailty, and Virtual Ward pathways. The service delivers urgent, responsive therapy interventions within people's own homes, offering an alternative to hospital admission and supporting timely discharge from acute services.
This role offers a unique opportunity to work both independently and as part of a supportive multidisciplinary team in a highly rewarding community setting. You will carry out holistic occupational therapy assessments in home environments, where clinical reasoning, risk management and person-centred decision-making are central to delivering high-quality care.

Main duties of the job
- Undertake comprehensive occupational therapy assessments within people's home environments.
- Assess activities of daily living (ADLs) and personal activities of daily living (PADLs) to determine support needs.
- Review and contribute to care packages, including identifying bridging care requirements.
- Complete falls assessments and environmental safety evaluations.
- Assess transfer, equipment and manual handling needs.
- Provide advice on safe transfer techniques and recommend appropriate equipment.
- Work collaboratively with the multidisciplinary team (MDT) and external partners to support safe and timely discharge planning.
- Contribute to staff training, supervision, and competency development where appropriate.
- Manage an autonomous caseload, prioritising urgent and complex referrals effectively.
- Maintain accurate clinical documentation and contribute to governance, audit and service improvement initiatives.
- Identify when individuals require ongoing social support and make appropriate referrals or signpost to local authority, community, and voluntary sector services.
- Work in partnership with social care teams to facilitate timely care packages and appropriate support based on functional need and risk
Having a full UK driving license and access to own transport for business purposes are essential.
