Job summary
An exciting opportunity has arisen for a compassionate and dynamic nurse to join a well-established nurse-led service that provides health care to people experiencing homelessness in the North West London. There are three full time posts available supporting the boroughs of Hounslow, Harrow and Hillingdon.
The posts are for 12 months.
The role will be supporting people experiencing homelessness on their discharge from hospital into a variety of accommodation settings. You will also be supporting people with complex needs to improve their health in the community and prevent hospital admissions.
You will be working closely with specialist hospital inclusion health teams across London and a variety of third sector partners including hostel staff, outreach services, and day centre staff.
The focus of the role will provide person- centred care, identifying unmet health needs and supporting the person to meet these needs. You will have good clinical skills and will provide hands on care as appropriate and manage a caseload.
The roles sit within the Homeless Health Service where you will be supported by an experienced nursing team, lead nurse practitioners and the Nurse Consultant.
Main duties of the job
We are looking for non-judgemental, skilled and dynamic nurse with a commitment to providing high standards of care for this vulnerable group. You will need to be flexible, an excellent problem solver, and have the ability to work autonomously and also as part of a team.
Candidates will have excellent communication skills and experience working in the community would be beneficial. The job is Monday to Friday.
To use advanced nursing skills and knowledge to provide autonomous holistic assessment, diagnosis and management of homeless patients presenting with acute undifferentiated and undiagnosed conditions and injuries, complex health needs and/or long term conditions.
To provide complete episodes of care based on the best possible evidence and work within clinical guidelines, utilising expert judgement to work outside these frameworks when necessary.
To provide advanced nursing care to people experiencing homelessness both in a clinic setting and during outreach to the street and other locations.
To work in partnership with other community teams / voluntary organisations in order to ensure the seamless delivery of care to the homeless population and proactive engagement with underserved groups.
To act as an advocate for people experiencing homelessness, assisting patients to overcome barriers to healthcare and supporting their own choices in regards to their care whilst promoting independence and self-care.
