Job summary
We are looking to recruit a motivated Band 6 Physiotherapist to join the Urgent Community Response (Rapid Response) Team within the Harrow Integrated Care Team.
You will work autonomously in the community, providing rapid assessment and intervention for patients referred from primary care, the London Ambulance Service, Accident and Emergency departments and other community services. The service aims to respond within two hours of referral, delivering timely, high quality care to prevent unnecessary hospital admissions.
This role offers opportunities to develop both clinical and leadership skills, with access to additional training such as venepuncture and catheterisation. You will work closely with multidisciplinary colleagues to support safe patient care, including follow up visits, escalation of deteriorating patients, and supporting safe discharge from hospital.
Main duties of the job

Deliver high quality, evidence based care within the Urgent Community Response service, respecting patients' preferences, dignity, and cultural or religious needs

Work autonomously to assess, treat and manage patients in the community, using clinical judgement to support safe decision making

Contribute to the continuous development of the service through quality improvement and best practice

Identify and respond appropriately to patients who are deteriorating, escalating concerns in line with service pathways

Develop clinical and leadership skills, including undertaking additional training such as venepuncture and catheterisation

Work flexibly across the service operating hours, supporting delivery of care seven days a week (Monday to Sunday from 8:00am to 22:00)

Hold a full, valid UK driving licence and have access to a vehicle for business use (reasonable adjustments will be considered in line with the Equality Act 2010)
