Job summary

The NHS Central London Community Healthcare (CLCH) Trust is seeking a motivated and enthusiastic pharmacist to join the Community Diabetes Team. This specialist clinical role delivers high-quality, patient-centred diabetes care within community settings.

The post holder will provide expert pharmaceutical input, focusing on medicines optimisation, safety, and personalised care for people with complex diabetes needs. The role aims to improve glycaemic control, reduce health inequalities, and prevent complications.

Working within a multidisciplinary diabetes team, the Community Diabetes Pharmacist will collaborate with nurses, dietitians, consultants, GPs, and other health professionals to deliver integrated care closer to home. The role also contributes to clinical service delivery, patient education, quality improvement, service development, and workforce training, supporting CLCH's commitment to high standards of governance and population health management.

Main duties of the job

As a Community Diabetes Pharmacist, you will join an award-winning team dedicated to improving care for people living with diabetes. You will provide specialist clinical pharmacy input to support the safe, effective, and evidence-based use of medicines across community and primary care.

  • The role involves structured medication reviews, optimising treatment regimens, and supporting the initiation, adjustment, and monitoring of diabetes therapies, including injectable treatments. An independent prescribing qualification is essential for this role.
  • You will provide education and counselling to patients, carers, and families to promote self-management and medicines understanding, while working collaboratively with a multidisciplinary team to deliver integrated care, including clinics, virtual consultations, and outreach services.
  • The post holder will manage medicines-related risks, contribute to clinical governance, audits, quality improvement, and service development, and support the education and training of colleagues and learners.
  • You will maintain accurate clinical documentation and continue to develop professionally through CPD in line with GPhC and Trust standards.
